Effective mental health management requires coordinated planning with a variety of healthcare professionals, which can often feel fragmented. To address this, modern electronic health records should shift their focus from merely documenting medical history to emphasizing a comprehensive mental health care plan tailored for each individual.

Care Plan 365 is a comprehensive, plan-centric clinical and practice management platform (web app https://app.cp365.health/) designed to facilitate collaboration among mental health professionals—including General Practitioners, psychologists, psychiatrists, social workers, nurse practitioners, and nurses—and their patients via a dedicated mobile application. This platform supports the co-design of personalized care plans funded by Medicare Australia.

CONNECT WITH YOUR CARE TEAM TO MONITOR YOUR MENTAL HEALTH

Upon your healthcare provider’s invitation, you are encouraged to download and register with the Care Plan 365 App. The app seamlessly integrates with your provider’s web portal, enabling an interconnected approach to your mental health care:

• Prior to appointments, utilize the following features within Care Plan 365:
  • Symptom Checker: complete an online assessment
  • Stressors List: identify life areas that contribute to your stress
  • Goal Setting: establish and share your short-term and long-term mental health objectives with your provider
  • Information Sharing: send your data directly to your care team and collaborate on the platform
• Attend scheduled appointments to review and update your mental health care plan regularly.
• Utilize your Medicare-funded care plan to access a range of services including psychologists, social workers, occupational therapists, and psychiatrists via GP referrals.
• Manage Tasks & Track Progress:
  • View and complete assigned tasks with your mental health team
  • Monitor symptoms and progress towards your goals collaboratively with your providers

WHO CAN ACCESS CARE PLAN 365?

The platform is available at no cost to all residents of Australia. However, access requires an invitation from a healthcare provider who uses the system, as it is designed for collaborative care planning. User registration is contingent upon being enrolled in a care team monitored through the platform; without this connection, the app cannot be utilized independently.
